How should forms look if you want to use Adobe Acrobat Pro to convert them to PDF forms with fields that can be filled in and saved?
After a form is made fillable, people can open your form on their computer, fill in the blanks, save the completed form to their computer, and email the completed form to you. Using Adobe Acrobat Pro, it is relatively easy to make a form fillable, but the form must look right to begin with.
FIELDS
The blank lines (fields) in your form should have a name in front of each entry.
Each field should have its own name, one line per name. For example:
City _______________________ State ____ Zip _______________
Don't put multiple names with only one blank line. Not:
City, State, Zip ______________________________________________
Don't put a single line under everything. Not:
City State Zip .
Don't put a single name with multiple lines. Not:
Address __________________________
_________________________________
_________________________________

CHECK BOXES
Chek boxes should not appear on your form to start with, just the field names. That is, the check boxes will be added using Adobe Acrobat Pro. To start with, all you need on your form are empty spaces where the check boxes will go. (Leave plenty of space.) For example:
Visa MasterCard Discover
but not:
Visa___ MasterCard___ Discover__
